The London Borough of Hammersmith & Fulham (the “Council”), in conjunction with the Hammersmith and Fulham Primary Care Trust (PCT) invites expressions of interest from suitably qualified and experienced organisations for the provision of a outreach support services with a minimum of three accessible hubs in the North, Centre and South of the Borough.

Organisations wishing to express an interest, request a pre-qualification questionnaire or register for a place at the provider briefing session on 25 August 2009 should do so by email to Patrick.Palmer@lbhf.gov.uk.

Expressions of interest must be received by the Council on or before 12:00 noon on Tuesday 02 September 2009 <strong?12:00 strong 2009. Requests received after this time and date will not be considered.
