First Contact Checklist Signposting and Community Agents

Hertfordshire County Council is seeking organisations to provide a new service in Hertfordshire that has three elements – First Contact Signposting; Community Agents and Health Trainers. The purpose of this service is to ensure older and vulnerable people in Hertfordshire have timely access to highquality information and a wide range of services and activities that enables them to make informed choices about their current and future needs; helps improve their physical health and mental wellbeing and enables them to remain independent at home for as long as they wish or are able.

The planned start date is March 2010 and the contract will be awarded for a period of two years with an option to extend for a further two years, subject to satisfactory performance and funding availability.

Organisations with a demonstrable track record in delivering services of this type and are able to deliver a countywide service are invited to submit an Expression of Interest in writing or e-mail.
