A multi-disciplinary service has been launched by Highland Council to tackle delayed discharge.
The 'rapid response team', based at the Royal Infirmary in Inverness, is made up of staff from social work and health. The team provides intensive support to people who develop serious health problems to prevent premature admission to hospital in the first place as well as around-the-clock, four-week support packages to help those being discharged from hospital care.
Sally Riddell, head of client services for Highland Council’s social work service, said: "This team limits the number of professional contacts and assessments that people are subjected to, promotes teamwork across traditional service boundaries and ensures that people get the right level of care at the time when they need it most."
