Local government ombudsman Patricia Thomas has criticised Manchester City Council for failing to have a "properly thought out strategy", following the death of a schoolgirl from a drug overdose on open ground in Manchester.
The council had received a letter from the girl's former local authority, West Lothian Council, stating she was on a supervision requirement and had difficulties, including non-attendance at school.
The council's decision to file this letter and mark it "no further action" amounted to maladministration, according to the report.
Although assistance was provided by the council's education and social services department this was not co-ordinated with other agencies.
The council agreed to pay £10,000 to the girl's mother and has now put in place new procedures for dealing with cases referred from Scotland, and altered its case recording system.
