Powys Area Child Protection Committee in Wales has ordered an independent investigation after a baby girl, who was removed from the social services at-risk list, was subjected to a series of violent assaults.
Chelsea Roberts was found to have suffered 27 different fractures and her stepfather, Tommy Roberts, was sentenced to four-and-a-half years in prison by a court in Merthyr Tydfil last week for the assaults.
Health and social workers from Powys had visited the child's home on a number of occasions but were told by her parents that she had injured herself when her cot collapsed. The agencies involved in the case predicted in a joint statement that the outcome of the serious case review was "likely to concentrate on professional practice and inter-agency communication".
