The inquest into the death of 16-year-old Gareth Price at Lancaster Farms young offender institution begins today.
Gareth was found hanging in his cell at the YOI in January 2005.
The five-week inquest in Lancaster will examine the suitability of Gareth's placement at the YOI, the role of youth offending teams involved in his case and whether there was a failure to ensure that key remand planning meetings took place.
The inquest will also look at why Gareth's family were never informed he was self-harming in prison and what action was taken to protect him.
It is the fourth inquest into deaths in young offender institutions this year, following those of Adam Rickwood, 14, the youngest person to die in modern penal history, 15-year-old Gareth Myatt and 18-year-old Daniel Nelson.
