The National Transition Support Team is a key part of delivering Aiming Higher for Disabled Children, and its website pulls together all the relevant documentation about best practice on supporting children into adulthood. The site itself is rather clunkily academic, but the resources section provides a useful repository of documents, particularly the TransMap, which highlights the best practice and developments in ensuring smooth transitions. Thirteen comprehensive case studies document the best practice to follow, and there's also a discussion forum to pull together new thinking on transition.
