Hospital Social Work - Best Practice and Caseload Management

As a, until now, community team social worker (adults with PD and older people) I'm about to make the transition to hospital social work and would appreciate any guidance from those already in post.

I have done many a hospital discharge but I appreciate being a hospital social worker is a different experience - certainly a faster pace than I might be used to. My attitude is positive though, I'm confident I will find my feet and support my clients well.

Do you do anything in particular to keep you on track and your caseload manageable? Any hints about how to keep the process streamline and ensure, first and foremost, the discharge is a safe and supported one?
