Government older people’s tsar Professor Ian Philp today launches a blueprint to shift NHS and social care services towards early intervention and supporting people with long-term conditions.
Philp, the Department of Health’s national director for older people will cite examples of good practice in England which he says should be rolled out across the country.
He said: “I am recommending a five-point plan to change older people¹s services for the better: early intervention, long-term conditions management, early supported discharge, acute hospital care and partnership working.”
The vision matches that of last year’s health and social care white paper, but this has proved difficult to achieve due to budget constraints in councils, primary care trusts and NHS trusts.
