If so then we want to hear from you. Lincolnshire County Council's Adult Social Care Hospital Team is seeking a Lead Practitioner to lead and manage a team of Social Workers and Community Care Officers in enabling and facilitating safe and timely discharges from Pilgrim Hospital Boston.

Along with a competitive salary of £39,880 - £44,863 + 15% Attraction Bonus, we are offering a comprehensive benefits package, contributory pension, career development pathway and up to 32 days holiday with the option to buy more.

It is essential that you hold a professional Social Work, Nursing or Occupational Therapy qualification and have current registration with Social Work England, NMC or HCPC (please quote you registration number/PIN on your cover letter). You should be able to demonstrate commitment to continuous professional development and to work as part of a team, learning from and contributing to the development of others.

Lead Practitioners are expected to support other hospital sites as and when required

About The Role

You will play a key role in supporting teams and maintaining relationships with Partners across the System including Neighbourhood Teams. Our partnerships are of great importance to us and are critical to achieving sustainable outcomes and positive effective outcomes for the people of Lincolnshire.  If you have a proven track record in building and maintaining professional relationships both within Adult care and with the health sector then this could be the role for you. 

You will be responsible for the management and leadership of the team and responsible for achieving timely and planned discharges from hospitals.

About you

You will need to have significant post qualification practice experience of working in a health or social care environment, along with;

  • Being able to demonstrate a sound knowledge of the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation underpinning statutory Adult Care Practice
  • Experience and knowledge of safeguarding in practice.
  • Ability to lead a team in a fast paced environment
  • Experience of working in a multidisciplinary setting and maintaining relationship with internal and external partners.
  • Proven background of dealing with people from all walks of life, dealing with the unexpected and providing advice, reassurance and help to find solutions within your team
  • Ability to provide and report information in a timely, efficient and accurate manner to line manager and senior management
  • An ability to be able to manage a team undertaking complex case work.
  • Participate in Quality Practice audits to a high standard

The successful candidate will need to demonstrate excellent problem solving skills and be able to deal with, and manage stressful and crisis situations. You will need to support your teams to be skilled in meeting peoples assessed outcomes, be committed to supporting effective multi-agency working with partner organisations and do so in a timely manner. You will also be expected to support and work alongside other managers in the Hospital Team being part of a 7 day working system which includes the duty manager rota covering weekends and bank holidays.
