H9 £32,234 - £34,728 pa – 3 posts.

M1 £35,745 - £39,880 pa - 1 post.


Hours: Full time, 37 hours per week over 4 week rolling period

Weekend Working requirement: 1 weekend in 4. You will receive 2 days off in the week when working a weekend.

Hours: 08:00 - 18:30 

Contract: Fixed term contract to end January 2023,   with the possibility of extending. Secondments will be considered

Job Duties:

New Early Intervention Vehicle Service

Hertfordshire Adult Social Care Services are looking for a dynamic, forward thinking Occupational Therapists and Social Workers to work in the newly established Emergency Intervention Vehicle Service supporting Older People in West Hertfordshire.  

This new and exciting initiative will deliver crisis management to individuals to prevent hospital admission and reduce the risks of social care placements. You will work in collaboration with Central London Community Health Service (CLCH) Paramedics and nurses to provide holistic assessments in responding to non-life-threatening priority calls from people in their own homes or in Care. In addition to these duties the M1 practitioner will be required to take responsibility for supervising colleagues and other additional duties supporting the team manager

 You will hold a relevant professional qualification and hold up to date registration for practice with your professional body. You will have experience in working with the older people and their carers. You will possess some knowledge of Social Care Legislation and have good understanding of the safeguarding policies governing provision of services to older people demonstrable abilities in your in day to day practice.

The Early Intervention Vehicle Service covers the whole of West Hertfordshire.  Practitioners will be expected to take positive risks and work creatively and flexibly to enable older people in to remain in their own home following an acute episode. You will work closely with the CLCH Rapid Response team and develop strong relationships with Adult Care Services colleagues.  You are also expected to liaise with other local health, social care and voluntary services to promote longer term recovery.  Professionals have immediate access to assistive equipment and will need to identify further needs and making the necessary onward referrals. 

This role will suit those interested in working flexibly over a 3 - 4 day rolling week. It is well suited to assertive professionals, looking for a unique challenge, with sound crisis assessment skills and a passion for working with clients to optimise their wellbeing in their own home. Staff can cover extra shifts if they wish.
