About The Service

This is an exciting time to join Cardiff, which is an Authority on the implementation journey that uses the Signs of Safety approach to focus on the support of our Children.

Intake and Assessment is a fast paced and exciting team, due to Cardiff’s demography the work is diverse and progressive. Practitioners undertake Well-being and S47 assessments and provide short term interventions to support families to increase safety within the home. Practitioners hold cases for a short period of time; long term work is generally transferred at the first planning meeting.

The Team are on a duty rota for one week in every three, meaning you have the opportunity to plan and record your work in a structured and organised way.

The posts are permanent and will be based in the Initial Response Team, within Intake & Assessment.

Social Workers may also be involved in care and private law proceedings. Cardiff Intake and Assessment Teams are established with experienced staff who support each other during challenging times. Cardiff works creatively under the Signs of Safety model for the best possible outcomes for families. What We Are Looking For From You

The successful candidate will have experience of assessment, care planning, person centre planning and care management skills working actively with all partners to identify and meet individuals care support needs.

You will be a registered Social Worker and have excellent skills in assessment and the ability to write concise records and reports. You will be a good team player, have excellent communication and organisation skills and have IT skills.

Additional information

With effect from 1st April 2022, this role attracts a market supplement of £5,000 (full time equivalent). (£39,373 – £43,553) This payment is reviewed on a 12 monthly basis.

Cardiff Council Adult Services support nearly 5000 individuals to live well, offering them a person-centred support structure to ensure they can meet their own needs and outcomes. Cardiff Council Adult Services consists of three social work teams:

Adult Community Services is the largest team in Cardiff supporting anyone over the age of 18 with physical disabilities and older people.  The teams are made up of community and hospital based Social Workers who offer services for individuals and families from the first time they need support from Adult Social Services throughout their journey with us.  Working alongside a wider multi-disciplinary team, including Occupational Therapy teams, Social Work Assistants, Day Centre teams and Health colleagues we find solutions to best support individuals to achieve their outcomes.  We have contractual relationships with a wide range of home care providers, offering care and support throughout Cardiff and residential care homes, ensuring that all options can be available to best meet an individual’s needs.

Our Mental Health and Specialist Support Services support a range of individuals including people experiencing mental health difficulties, drug and alcohol misuse issues and homelessness.  The teams work in collaboration with a number of partners including CAVUHB and third sector organisations to provide services to individuals and families to help them negotiate the challenges they face and provide ongoing support where needed.

The Learning Disabilities teams provide social work advice, information, and support services to adults with Learning Disabilities and their carers, working closely with wider multi-disciplinary teams to support ongoing complex needs.  The team focuses on the strengths of the people worked with and opportunities are sought to support people to feel valued and included in their communities. A strength-based approach is used and the teams are committed to enabling people with learning disabilities to maximise independence, choice and control.

Salary
A competitive salary in the context of a fair pay structure with annual pay reviews and incremental progression. Each Social Worker post (including Team & Service Managers) receives a £3,000 market supplement (full-time equivalent), reviewed annually.
