The changes to Serious Case Review guidance recently proposed by the government still only require the publication of the executive summaries of the documents. Community Care believes reviews need to be published anonymously in full in order to maximise learning. Here we call on you to let the government know if you too believe reviews should be published in full.

Dear Mr Balls,
The changes you propose to Serious Case Reviews in the revised version of Chapter 8 of the Working Together to Safeguard Children guidance would still only see the publication of the executive summaries of the documents. Serious Case Reviews need to be published anonymously in full to maximise opportunities for professionals to learn from mistakes. Copies of the report also need to be made freely available. Anything less than full publication will represent a missed opportunity and severely limit the potential for learning reviews could provide.
