A social worker was “unqualified” for their job, having missed signs of abuse to a three-year-old girl in Cumbria, according to a serious case review.
The child’s family moved to Cumbria from another local authority, which was not named by the review, and lived there for 19 months before the injuries were identified.
The review said Cumbria Council had experienced a “failure to collate a holistic picture prior to a visit and the lack of managerial oversight of the process”.
The council has since restructured its response services so that initial assessments are carried out by qualified social workers who are supervised by experienced managers.
An auditing framework schedule has also been developed and presented to the local safeguarding children board. The council has said regular auditing and management oversight will ensure that assessments are carried out “to a high quality”.
The report emphasised that both parents have been jailed, the father for abuse and the mother for concealing abuse. The daughter is now living with relatives and is said to be “thriving”.
