HampshireHampshire County Council requires delivery of services providing community and accommodation based housing related support to improve outcomes for vulnerable people who are homeless or at risk of homelessness. This tender will be for 5 Lots with each Lot having its own contract.

The Services will provide a model of service delivery based on the staged model and in partnership with other Providers of local services for the target service user group. The model implemented will meet the diverse needs of people in need of housing related support and Service Providers will develop innovative solutions that maximise the use of resources, avoid duplication, reduce bureaucracy and unnecessary process, and improve service access.

Providing support to vulnerable people

All services will provide flexible and responsive support to vulnerable people who are in need of support because they are homeless or at risk of becoming homeless. This may include but not be limited to, people with substance misuse issues (including dependency and dual diagnosis), mental health problems, physical disabilities or long term physical health conditions, learning disabilities (where service users do not reach the threshold for adult social care).

Services will focus on preventing escalation of need by working in partnership with a wide range of other agencies and supporting service users to develop resilience, sustain independent living and avoid repeat episodes of homelessness and/or other crises that jeopardise their housing situation.

Providers will also be expected to proactively manage expectations of service users and improve partnership working with both statutory and third sector organisations. Contracts will be awarded to organisations that demonstrate creative solutions to maximising resources and are proactive in addressing the barriers to effective service delivery.

The contract will be for a period of three years with an option to extend for a period or periods not exceeding two years in total. The combined contract value for all 5 Lots will be around £10,253,994.00 for three years. If the option to extend for the maximum two years is exercised the combined contract value for all 5 Lots will be around £17,089,990.00. Anticipated contract commencement will be 1st April 2016.
